After graduating from Frederick Community College in 1987 as a Registered Nurse (R.N.), Tracey worked for the next decade in both the E.R. and I.C.U. at the University of Maryland Shock Trauma Center, as well as in Hospice and Home Health throughout the tri-state area. In 1997 she earned a master’s degree in Family Medicine with a specialization in gynecological and women’s health, eventually fulfilling her dream of opening her own practice in 2004 in the heart of Boonsboro, MD. Tracey has a deep passion for family medicine and the impact sustained, comprehensive healthcare can have on the lives of her patients. She has also maintained her commitment to Advanced Practice Nursing and mentors at least one or two students a year. She has been in private practice since 1997 and continues to love every minute of it.
Tracey is very proud of her wonderful children (two of whom you might see around the office!) and four senior dogs who love her dearly. In her spare time she enjoys gardening, hiking, cooking, and scheming new ways to adopt even more senior dogs. Habla español.
Kelly is a life-long resident of the Brunswick MD area. She is Board certified as an Adult/Geriatric nurse practitioner. She obtained an Associate Degree in nursing from FCC in 1987, Bachelor of Science in Nursing from UMBC in 1995 and Master of Science as a nurse practitioner from University of Maryland in 1997. She completed her Doctor of Nursing Practice (DNP) degree from University of Maryland in 2013, specializing in leadership, and a PhD in Nursing Education from Indiana University of Pennsylvania in 2022. Kelly works part time in the office and full time as a professor of nursing at Shepherd University in Shepherdstown WV. In her free time, she enjoys working with her horses, painting, and being outdoors.
After graduating from nursing school with her RN, Jill started her career as an emergency room nurse at a busy urban hospital in Prince George's County, MD. Jill spent over 10 years as a nurse in the emergency department setting in both Prince George's County and Washington, D.C., and has experience working with patients experiencing both minor injuries and illness, to patients who are highly complex and critically ill. In 2021, Jill returned to school to pursue her master’s degree and graduated with board certification as a Family Nurse Practitioner. Jill has practiced in an urgent care and primary care setting as well as a virtual setting as a Nurse Practitioner. Jill is passionate about improving access to quality healthcare services, so those who have limited access to healthcare are able to connect with a provider. Jill is excited to meet everyone and cannot wait to form relationships with the South Mountain Community Health patients and their families/caregivers. Jill and her family enjoy a busy life in a small-town setting. Homework, sports, and the occasional nature outing occupy most of the days, but they also enjoy traveling and being active participants within their community.
Joanna recently graduated from the University of Maryland with her family nurse practitioner degree and is currently part of the Johns Hopkins SNAPT fellowship. She originally received her nursing degree from the University of Maryland. With over 6 years of experience as a registered nurse, she has worked in critical care and post-surgical settings, and has spent 2 years as a clinical and lab nurse instructor at Frederick Community College. Additionally, she has served as an adjunct professor at the University of Maryland, teaching advanced health assessment to other nurse practitioners.
A Maryland native, Joanna was born in Baltimore City and grew up in Prince George's County. She now resides in Frederick County with her husband and two dogs, Ollie and Griffin. In her free time, she enjoys swimming, kayaking, and hiking.
OFFICE STAFF
Mariana joined South Mountain Community Health in 2018. Her goal is to continue pursuing her A.S. in nursing. Mariana grew up in Washington County and hopes to be able to give back to her community. Outside of work, she enjoys spending time with her friends and family, traveling, relaxing at home, and always learning something new. She is a native Spanish speaker.
Angela graduated from South Hagerstown High School and initially studied business management at Saint Leo University. However, she ultimately redirected her path towards the healthcare field, driven by her passion for making a difference in people's lives. Outside of work, Angela enjoys outdoor activities like hiking, she is an avid dog lover and takes great pleasure in caring for her furry companion, and values spending quality time with friends and family.
Katie is a mother to her wonderful 4-year-old daughter and happily married for 10 years. With a background as a pharmacy technician for a decade, she is now embarking on a new journey pursuing her passion for healthcare by studying Health Information Management. She is an avid reader, and you'll often find her immersed in the pages of a good book. When she is not hitting the books, she loves nothing more than spending quality time with her family and exploring the great outdoors through camping adventures. “Life is a beautiful journey, and I'm excited to see where it takes me next!”
Living in Brunswick, MD, with her one-year-old son, boyfriend, and several cats, Brittany brings her experience and dedication to her role at SMCH. Before joining the team, she worked as a dental assistant for three years. After her son was born, she decided to pursue her dreams of getting her MA certification and becoming an RN. Outside of work, she enjoys spending time with her family, especially at their cabin in WV.
Liberty is our dedicated and meticulous phlebotomist at South Mountain Community Health on behalf of Quest Diagnostics. With over 12 years of experience drawing blood from patients of all ages, Liberty's main task is collecting blood samples. But more importantly, her top priority is ensuring your comfort during your visit. She believes in providing care, understanding, and empathy to all patients, following the motto of treating others as she would like to be treated. When you're with Liberty, your arm becomes her arm every time!
Chase graduated from Governor Thomas Johnson High School in 2018 and spent two years studying kinesiology and behavioral health at The University of Maryland, College Park before joining South Mountain Community Health. You won’t see him in office because his position is fully remote assisting the back office with business and administrative tasks. In his free time, he loves spending time with his family and his girlfriend, visiting local national parks, and reading up on new ideas for bettering our practice.
